NAHS, in conjunction with the Adams County Medical Foundation, is administering the OHYES! survey to all NAHS students in grades 7 - 12. The foundation uses the data to improve health services in Adams County. The online survey will be given on May 12, 2021.
Parents may opt their student out of the survey by completing the form below and returning it to the school office prior to May 12, 2021.
